Overview
The Palliative Outreach and Consult Team (POCT) is a team of doctors and nurses who specialize in providing quality palliative care and symptom management.
The team can be accessed by anyone in the organization, including all patients and residents.
POCT members seek to deliver person-centred care. They focus on enhancing the quality of life of those living with advanced chronic and/or life-threatening illnesses. POCT strives to ensure that all people have access to excellent and appropriate care.
Where we fit
We work at each site as part of a team to support you in navigating your care over time. Our interdisciplinary team consists of your doctor, nurses, care aides, pharmacist, rehabilitation therapists and social workers.

Support for Indigenous Peoples

POCT Triage nurse
The nurse will talk to the patient's Most Responsible Provider (MRP) in the hospital about the consultation request. The MRP is the physician who is primarily responsible for a patient’s care.
Hours
Monday to Friday
8:00 a.m. — 4:00 p.m.
Medical & professional referrals
Primary care doctors and community health providers can send their patients for consultation. They can refer to either St. Paul’s Hospital and Mount Saint Joseph Hospital via the contact numbers above.
Members of the palliative outreach and consult team (POCT) see the referrals. We offer consultation in many languages with professional translation services, as required.
