Value-based health care

VBHC is an approach focused on achieving health outcomes that matter to patients and families.

Value-Based Health Care (VBHC) is an approach focused on achieving health outcomes that matter to patients and families. It’s a key goal in our Mission: Forward strategic plan which aims to transform the way we provide care.

For patients, health outcomes that matter means being able to do things that "let me be me" (capability), "feel better" (comfort) and reducing the chaos of managing a chronic condition so "I can live my best life" (calm). As care providers, this is what we want for patients too.

Enabling VBHC at Providence

We’ve assembled a team at Providence to help lay the foundations for VBHC. This team is developing roadmaps and building tools to support care teams looking to embed a value-based approach in their practice.

VBHC is being implemented in a few areas of the current St. Paul’s Hospital to get ready for the new hospital  opening in 2027. These projects are helping us fine-tune our processes so we can expand to other areas across Providence, particularly for complex chronic diseases.
