Person- & family-centred care

We strive to provide the best possible care by enhancing engagement with patients, residents and families.

At Providence Health Care, we believe in partnering with those we serve and their families (as defined by them) in their own care. These partnerships are the path to better patient outcomes and experience.

We believe in creating relationships built on trust and respect. These relationships can reduce the power imbalance that has traditionally existed between caregivers and patients, where caregivers have more authority and control. In these new relationships, caregivers and patients have an equal say in the patient's care.

By partnering with those we serve, we can offer care that is valuable and valued by them.

And what matters to them is what matters most to us. And that is better care.

Our approach

Person and Family-Centered Care (PFCC) is an approach to the planning, delivery and evaluation of health care, done in partnership with health care providers, patients, residents and families.

At Providence, we use a person-centred approach to deliver care. We also use this approach in education, research, quality improvement projects and in the  planning for the new St. Paul's Hospital. PFCC is a foundational principle of our organization and helps guide our actions and decisions.

At Providence, PFCC looks like:

  • Collaborative decision-making
  • Patient and Family Partners embedded at all levels of the organization
  • Family Presence Policy
  • Patient Health Education Materials  that are created with patient and family partners
  • Respect for Diversity: we strive to provide inclusive and culturally competent care.
  • Supporting the people we serve, and their families, with tools and resources to be full partners in their own care.

Together, we can create a supportive and healing environment that places the person and their family at the center of care. That’s How you want to be treated.
