About this space

The Vancouver Police Foundation Transitional Care Centre (VPF TCC) is part of a group of wrap-around support services for people with substance use and/or mental health challenges.

The VPF TCC has no official capacity as a clinical treatment facility. Rather its objective is to ensure individuals are given the opportunity to move into the next level of service.

It is a non-clinical space o­ffering short term, transitional accommodation for patients discharged from St. Paul’s Hospital who are in need of:

  • Short-term shelter
  • Assistance that would help connect to community services not available at the time of discharge (e.g. overnight, weekends, statutory holidays)
  • A safe place alternative to the street, post-discharge, to bridge to existing care and/or existing safe place.

Clients are provided with access to primary care, income support, counselling, specialist care, Indigenous peer support and services, and referrals to community services.

Accessing the VPF TCC

To access this transition space, discharging patients must be referred by a Providence Health Care clinician or staff member.
