Preparing for your visit

An internist or cardiologist must refer you to this clinic. Once the care team reviews and accepts the referral, the clinic will make an appointment and contact you with the information. At your first visits, you will meet with our team for further consultation on your advanced heart failure.

Getting ready

What to bring:

  • BC Services Card
  • A list of the medications you are taking, including herbal supplements
  • Summary of your medical history, if you have one
  • A list of who your doctors are
  • A record of your weight for the last month (if you have it)
  • A record of your blood pressure and blood sugar levels for the last month (if you have them)
  • A list of questions you may have

Cancellation policy

To cancel or change your appointment, please call the clinic within 48 hours of your appointment. This allows us time to schedule other patients.

If you feel acutely unwell and your symptoms worsen, please see your primary care practitioner or visit the emergency room and notify the clinic.

Transportation & parking

Please allow about 30 minutes before your appointment to get from your car or transit to the clinic area. If you are new to the clinic and need directions, when you arrive at St. Paul’s Hospital, ask a volunteer or reception for directions to our front door. Please remember the name of the clinic to get the best help.
