Preparing for your visit

Please bring with you to all your appointments:

  • Your BC Services Card
  • A list of current medications, including herbal supplements
  • A summary of your medical history if you have it
  • A list of all of your doctors and specialists
  • Your last recorded weight
  • A record of your blood pressure and sugar levels from the last month, if you have it
  • A list of questions you may have for your health care team

Cancellation policy

To cancel or change your appointment, please call the clinic within 48 hours of your appointment. This allows us time to schedule other patients.

If you feel unwell and your symptoms worsen, please see your practitioner or visit the emergency room. Also, let our clinic know.

Transportation & parking

Remember to allow time (approximately 30 minutes) before your appointment to get from your car or transit to the clinic area. If you are new to the clinic and need directions when you arrive at St. Paul’s Hospital, ask a volunteer or reception for orders to our front door. Please remember the name of the clinic to get the best assistance (Post-Heart Transplant Clinic).
