Post-Heart Transplant Clinic

During your visit

During your visit, we will ensure you are stable after your heart transplantation. This includes reviewing your medications and any symptoms you may be experiencing. The care team will make any necessary changes of your medication as well as review any testing you had done since your last visit.

We will also teach you to recognize the signs of rejection, infection, or other post-transplant related symptoms and when to call for help.

If you have any questions and concerns, we will discuss these with you during your visits to the clinic. We will also provide a personalized care plan with regular surveillance testing to help you monitor your new heart. Our goal is to give you the support and education you need to live and enjoy your life after a heart transplant.
