Photo ID
Information about accessing and replacement of Providence photo ID.
Overview
School identification must be worn on the left upper chest and be visible at all times while on placement at PHC, preferably ID with a photo. Please refer to the Discipline-specific page to determine if you will be provided PHC ID.
Getting your PHC ID
If you require ID, please complete the PHC ID Access Request Form (accessible ONLY via a health authority workstation).
Lost or stolen ID
Contact the Photo ID department (or Security if after business hours) immediately to report and have your card deactivated.
