Through the innovative Hospital at Home program, we provide a new standard of care that enables patients to receive hospital-level treatment and support in the comfort and safety of their own home.
As a nurse with Hospital at Home, you'll provide high-quality patient-centered care to individuals in their own homes. You'll also be part of a collaborative, multidisciplinary team that provides both acute and chronic care management, delivering the same level of care that patients would receive in the hospital, but in a more personal setting.
