Digital Patient Questionnaires – Information for Patients

Providence Health Care (PHC) collects your personal information to provide health care and related services to you. PHC collects your personal information for these purposes under section 26(c) of the British Columbia Freedom of Information and Protection of Privacy Act (FIPPA).

What are digital patient questionnaires?

Digital patient questionnaires are health care forms your care team may ask you to fill in and submit online using a computer, smartphone or tablet. PHC is working in partnership with Thrive Health to send out digital questionnaires as an alternative to paper forms. Completing questionnaires electronically has benefits such as: 

  • Helping you and your health care team use your appointment time well, and make informed choices about your care.
  • Letting your health care team learn about your health history and health status at different time points.
  • Allowing you to complete questionnaires in a location that is convenient to you, at your own pace and with the help of a family member or friend (if needed).
  • Enabling your care team to use aggregated data for quality improvement.

Is there anything I need to do?

Completing digital patient questionnaires is optional. Let your care team know if you would prefer to complete questionnaires using another method to share information with them, such as paper, telephone or email.

If you consent to using the Thrive Health platform, your care team will send you an email asking you to set up an account. Once you have done that, your care team will be able to send you digital questionnaires and/or educational materials via the platform.

What if I still have questions?

  • For questions related to your health care, please contact your clinic. If you do not have their contact information, please call the PHC Switchboard at 604-806-9090.
  • For further information regarding the collection, use or disclosure of your personal information, please contact the PHC Information Access and Privacy Office at privacy@providencehealth.bc.ca.
  • For technical support with the Thrive Health platform, visit the support section of their website . You can search for articles or click the speech bubble at the bottom right to chat with a Support Agent.
