About this clinic

The Transitional Pain Clinic (TPC) is a short-term outpatient clinic. Its multidisciplinary approach provides surgical pain management services. Our purpose is to optimize pain control before and after surgery. We conduct appointments by phone and if needed, in person.

Services that we provide include:

  • Pre-operative consults with individuals who are at high risk for increased post-operative pain
  • Medication monitoring to safely taper opioids and optimize non-opioid medications
  • Education about pain science, breathing strategies and non-pharmacological pain self-management strategies
  • Supporting safe, gradual increases in physical function after surgery and return to daily activities

Reduce pain crises and unexpected emergency hospital visits. We provide early healthcare support after discharge by offering pain services 1-2 weeks from discharge.

Medical & professional referrals

  • Pre-operative patients: Referred by their surgeon, primary care provider or the Pre-Admission Clinic.
  • Post-Operative patients: Seen within 1-2 weeks of discharge from the hospital after surgery. They are referred by the following services:
    • Acute Pain Service (APS) or the clinical nurse specialist for pain
    • Surgical Services and primary care providers
