Going home checklist

The following checklist can help prepare for your return to home.

- Is a friend or family member able to pick you up? Depending on your procedure, you may not be able to drive or take public transit alone for 24 hours.
- Do you have your house keys with you?
- Is a family member, friend or your building manager able to assist?
- Do you have a change of clothing for your trip home?
- Is your kitchen stocked with food? It may be difficult, if not impossible, for you to do your own grocery shopping.
- Can a family member or friend help you with grocery shopping, or can you arrange for grocery delivery?
- Have you received instructions on how to care for yourself at home?
- Will you need help with day-to-day care, like bathing, dressing or cooking?
- Do you know what medications you need to take?
- Do you know how to take your medication, including how much and how often?
- Will you need a prescription filled before you go home?
- Will you need to order special equipment, such as a wheelchair?
- Do you know where to get this equipment?
- When is your follow-up appointment?
- Will you need further treatment at Mount Saint Joseph or will you see your family physician?
- Have these appointments been arranged or do you need to book them?
- Do you know who to call if you have any questions about your recovery?

After you are discharged, you may receive a Patient Satisfaction Survey.
