Safe reporting

Overview

Providence Health Care (PHC) is an open and accountable organization. We value respect, integrity, trust, and open discussion. We encourage you to speak up when things aren't right. Speaking up helps keep our patients, communities, and colleagues safe.

We have several options for you to tell us about issues or wrongdoing. These include:

Reporting to your manager
Reporting to Occupational Health and Safety (phcnobully@providencehealth.bc.ca)
Contacting the Patient Care Quality Office

We encourage you to use these options. If these options are not appropriate in the situation, you can use the Safe Reporting Hotline. The Safe Reporting Hotline is confidential and independent. We will keep your identity private (anonymous) to the fullest extent possible.

Who can make a report?

Anyone can report a known or suspected case of wrongdoing by PHC to the Safe Reporting Hotline.

- Theft, fraud, bribery or corruption
- Actions that go against the law or which do not comply with legal regulations
- Unethical or unprofessional conduct
- Unauthorized use, misuse, or waste of public funds or resources
- Non-compliance with procurement (purchasing) rules or other unfair trade practices
- Actions that go against PHC policies or procedures, internal financial controls, or audit procedures
- Continuing to act while in a real or perceived conflict of interest
- Mistreatment of patients, staff or members of the public
- Actions that pose a danger to patients, public health, or the environment

Other reporting options

Current and former employees may report serious or systemic wrongdoing under the Public Interest Disclosure Act (PIDA).
