Website brings help to chronic disease sufferers
A new website is helping patients with two or more chronic conditions, such as diabetes, heart, kidney or lung disease, monitor their health from home.
In a Simon Fraser University study investigating the effectiveness of Internet-based patient reporting, individuals enter measurements, such as their blood pressure or weight, into the My Health Connect portal, which uses an algorithm to determine if these values are outside the individual’s normal range. If they are, the program sends an automatic email to a nurse, who will phone the patient to offer counselling and advice on whether a doctor’s appointment is necessary. Read full story.
