Pharmacy
Pharmacists at PHC are responsible for the safe and appropriate use of medications to achieve optimal therapeutic outcomes that improve patients' quality of life. To carry out this function, they work closely with the health care team, contributing their knowledge of optimal drug therapy and the potential for adverse drug reactions, seeking the best balance of safety and effectiveness. Pharmacists assist in the coordination of patient therapy as they transition from the hospital to the community.
Pharmacy at PHC also serves outpatients, primarily the transplant and HIV populations, but also chemotherapy and hemosiderosis patients. Pharmacists also support the home IV-antibiotic program for adult patients in the city of Vancouver and ambulatory programs such as Healthy Heart and renal dialysis.
