Elder Care Ambulatory Clinic
Located at Mount Saint Joseph Hospital and St. Paul’s Hospital, the Elder Care Ambulatory Clinic provides outpatient services to patients 65 years and older with at least one of the following: multiple, complex, chronic medical problems; cognitive impairment; motivational problems; confused state; functional impairment (such as the ability to do self-care impaired mobility); social problems (such as tenuous living situation, caregiver burden).
Geriatric psychiatry service is also offered under the Elder Care Ambulatory Clinic. This service provides acute care and consultation, initial assessment, and follow-up care for ambulatory patients over the age of 65 who are experiencing complex psychiatric disorders.
Patients must be referred by home care or long-term care facilities, physicians, or family members along with agreement of the family physician.
