The Inner City Youth Program delivers a health experience for youth from ages 12 to 24 by providing access to primary care, mental health and substance use services, psychosocial rehab supports, peer support, and recreational activities. Based out of the Granville Youth Health Centre on Granville Street between Davie and Drake, the centre welcomes walk-ins on weekdays. Reporting to the Operations Leader, Inner City Youth Program, the Youth Housing Coordinator is responsible for coordinating placement of clients in housing. Liaises with BC Housing and other private housing providers (market housing) to source out and/or develop opportunities for supported housing. Receives housing referrals, reviews cases/clients files, gathers pertinent information including discussion with interdisciplinary team members, interviews clients to assess needs, gathers requirements and determines suitability for various housing options. Makes recommendations to others for appropriate housing based on client needs. Establishes and maintains a streamlined placement and application process and assists clients with completion of required documentation for subsidies. Makes home visits to ensure housing arrangements are in place according to plan; develops rapport with Landlords and troubleshoots and/or identifies issues and refers to Case Manager or others as required. Represents the housing program at meetings internal/external to the organization to gather information and explains program processes and procedures.

Ability to organize, prioritize and meet deadlines. 
Ability to operate a computer, utilize standard office desktop software and client care 
information systems. 
Physical ability to carry out the duties of the position.


Diploma from a recognized Social Services Support Worker educational program. Three (3) years’ recent, related experience, or an equivalent combination of education, training and experience.


Coordinates subsidized housing program for the Inner City Youth Program (ICYP). Liaises with BC Housing and other private housing providers to develop opportunities for dedicated units of supported housing and market housing with subsidies for clients. Establishes and maintains streamlined placement and application processes. 
Receives housing referrals; reviews cases/client files, gathers pertinent information, interviews clients to assess needs, gathers requirements and determines suitability for various housing options. Makes recommendations to others for appropriate housing based on client needs, special circumstances and/or conditions, etc. 
Establishes relationship with clients and landlords; identifies emergent issues and liaises with Care Managers and other members of the interdisciplinary team to resolve. Makes referrals to other housing programs. Acts as an advocate on behalf of clients with various individuals and agencies including landlords. Assists clients with problem solving in areas such as financial assistance, accessing community resources, etc. Ensures client is linked to services that address their needs. 
Assists clients with completion and submission of required documentation including forms for subsidies. 
Maintains budget information, statistical records and reports on various activities for the program; develops plan for subsidies and identifies cost savings opportunities, prepares cheque requisitions, monitors expenses against budget, identifies, investigates and reports variances to Operations Leader. 
Represents the program with internal and external stakeholders; explains housing program processes and procedures. Reviews and maintains related documentation such as progress reports, case plans etc. Makes arrangements for moving and storage of personal belongings. 
Carries out responsibilities in accordance with health and safety requirements.
