The Senior Clinical Planner - Resident Care Manager will play an integral role in the clinical development strategy of Seniors’ Care at Providence Health Care (PHC). In collaboration with the Seniors’ Care Team, the person in this role will lead and manage the clinical planning process for Seniors’ Care initiatives and projects across PHC’s long-term care and assisted living homes. Summary

Reporting to the Program Director, Seniors Care and working in consultation with the Resident Care Managers, Seniors Operations Leaders, and Program Specialists, the position provides leadership in planning and implementation of clinical and operational system design activities and the development and management of clinical transition plans to meet the vision and objectives of the Long Term Care and Assisted Living clinical program development strategy. Ensures facilitation of physician, resident, and family engagement; the development and implementation of comprehensive clinical services plans; the design and integration of comprehensive clinical services to achieve desired clinical outcomes, and the evaluation of overall clinical program performance.

Develops the related work plan(s) and coordinates and manages the system design streams of work from sub-projects/task groups as part of the overall integrated work plan. Provides effective project management leadership and guidance to ensure all aspects of assigned project work accountabilities are delivered to scope, high quality standards, schedule, timelines and budget as described in the various project plans relating to the overall project. Regularly tracks, monitors, and evaluates project performance and progress at all stages of planning and implementation to ensure high quality standards are consistently achieved in an effective and timely manner. Identifies, analyzes, problem-solves, and addresses risks and roadblocks, develops risk mitigation strategies, and makes recommendations regarding project scope changes. Ensures timely and appropriate escalation of problems and identified risks.

In addition, the position assumes the role of a Resident Care Manager for designated site(s) in the absence of Resident Care Managers and/or for specific project implementation including collaborating with Physician Leaders on program delivery, providing leadership and supervision to interdisciplinary teams, ensuring standard application of corporate policies and regulations, and clinical practice standards appropriate to the site(s), effective resource management and stakeholder relationship management, and participation in managerial on-call activities.

Skills

Broad knowledge of health care projects and relevant government procedures, standards and requirements in the long term care/seniors care sector.
Comprehensive knowledge of change management and project management principles, methodologies, and best practices, and ability to plan and coordinate major projects from conceptual design through to completion.
Ability to continuously identify and implement improvements in work systems to ensure that the highest possible level of quality service is achieved.
Ability to supervise, coach and mentor others.
Ability to coordinate, lead, and facilitate projects and related teams.

Education

A level of education training and experience equivalent to a Master’s Degree in Nursing, Allied Health Science, or Health Administration from an approved educational institution plus five (5) to seven (7) years’ recent, related clinical planning experience, including large project and or program management leadership (including financial) of complex strategic and operational multi-site projects and consultation processes with a wide range of stakeholder groups (including the public, senior and middle management, physicians and staff).

Duties

Supports the Program Director by leading and managing the clinical planning process for clinical initiatives/projects including the master planning for functional programs, design phase of project development and business case development by consulting and securing agreement from program users such as Resident Care Managers, Physicians, Staff, Site Councils, Residents, Families and other stakeholders on clinical needs, determining if clinical needs are appropriate, and coordinating reviews of these needs for each initiative.

Consults with clinical/research/ academic programs, long term care/hospital/community services and coordinating clinical teams to secure necessary information.

Works in collaboration with internal and external stakeholders to develop documents such as detailed project charters, business case and functional program development that incorporates the project proposals for submission to Senior Leadership, Boards, Ministry of Health, and other funders.

Facilitates the approval of major clinical initiatives and leads the preparation of appropriate business cases and documentation containing justification of proposed project, strategic option analyses, assessment of risks and impact, cost benefit analysis and other options for funding approval.

Develops the related work plan(s) and coordinates and manages the system design streams of work from sub-projects/ task groups as part of the overall integrated work plan. Meets with stakeholders to explain project parameters and seeks input to identify needs and skills required for the project team(s). In conjunction with Project leadership, develops, establishes, leads and guides the work of project teams on assigned projects. Coordinates the process of consultant selection, project tendering and various planning processes and assists and coordinates the process of design development and contract preparation. Establishes roles and responsibilities of team members, identifies skills required, communicates quality expectations and work plans and discusses timelines that must be met for the project(s).

Identifies risk mitigation strategies and monitors risk throughout the life cycle of the project. Prepares final reports and presentations for use by internal and external clients as required. Ensures timely and appropriate escalation of problems and identified risks

Executes project plan(s) according to project methodologies, best practices, and the approved project charter and ensures successful, coordinated, high quality, and timely completion of project components. Facilitates consensus with stakeholders, project teams and Project leadership as needed, and ensures readiness for project implementation.

Ensures effective and timely communication channels are in place to keep the Seniors Care leadership up to date on project team decisions, status, progress, and quality and/or risk issues.

Works in collaboration with stakeholders and leadership team to develop a process of evaluation for project outcomes, appropriate data collection and analysis. Regularly tracks, monitors, and evaluates project performance and progress at all stages of planning and implementation to ensure high quality standards are consistently achieved in an effective and timely manner.

Acts as a resource and provides support to teams, staff and consultants and ensures that high quality project management principles are adhered to throughout the project stages. Mediates to resolve specific issues including interpersonal conflicts.

Develops, manages, monitors, and adheres to allocated project budget(s). Liaises with the Project and other project leaders on variances and/or other issues.

Maintains a thorough understanding of available resources for projects and establishes productive vendor relationships as it relates to project management activities and needs.

Provides supervision, leadership and performance management to designated interdisciplinary team members, consultants, and work groups, by providing consistent and effective day-to-day management of the service(s), project(s) and the team(s) including coaching, mentoring, skill development and, where necessary, corrective action.

Creates an environment that embraces and facilitates interdisciplinary team learning, evaluation and continuous improvement.

Develops, recommends, and implements, in collaboration with Human Resource Representative(s), strategies for recruitment and retention of staff that support the goals and objectives of the clinical area(s) of responsibility.

Facilitates the development and implementation of orientation programs for new staff to provide the appropriate education in clinical practice routines, and PHC policies and procedures.

Develops, recommends, implements, and evaluates staffing patterns, rotations and vacation schedules to ensure sufficient staff to meet patient/resident care needs.

Participates as a member of the leadership team by performing related duties such as formulating employer demands for collective bargaining, developing strike contingency plans, representing the employer during grievance proceedings, participating in the managerial 24 hour call structure and facilitating and initiating service delivery change proposals.

Participates on a variety of committees and establishes positive working relationships in order to ensure successful outcomes and cooperation from others.
