Summary

Clinical & Systems Transformation (CST) is one of the largest healthcare projects in British Columbia’s history. It is a joint undertaking between Vancouver Coastal Health, the Provincial Health Services Authority and Providence Health Care to complete a significant transformation of clinical practices and systems. This is an exciting initiative that will deliver key benefits for our patients, healthcare providers and health organizations.

Reporting to the HIM Project Coordinator, Document Imaging/Forms Management, the Forms Clerk applies an advanced understanding of desktop applications and forms processing in using designated computer applications to create new and/or convert existing healthcare forms into electronic template formats. Gathers form requirements from team members and creates electronic versions in accordance with Health Information Management (HIM), Clinical & Systems Transformation and other relevant departmental guidelines and/or protocols. Participates on internal and external committees such as organizational forms committees to gather and relay information for approved forms. Maintains Forms Library as required utilizing electronic health record systems. Creates bar codes in the applicable system and modifies as required. Maintains and develops chart pack types and form order within the types resulting from communications with applicable clinical/project/clerical staff. Liaises with applicable external vendors on issues as required.

Skills

Education

Grade 12, graphics or forms design courses, plus two year’s recent, related experience, or an equivalent combination of education, training and experience

Duties

Participates in the overall development of PHC/PHSA/VCH forms related to electronic health records by methods such as gathering form requirements from CST project teams members, providing input for forms-related issues related to health records, ensuring needs are met from a Health Information Management perspective in accordance with departmental guidelines and/or protocols and industry standards.

Develops forms, creates and/or maintains document layout definitions, makes modifications, and ensures forms fit within electronic health record systems. Participates in form testing and implementation. Maintains systems for notifying and updating user departments on form updates/revisions.

Maintains Forms Library as required utilizing electronic health record systems. Creates bar codes in the system and modifies as required.

Maintains and develops chart pack types and form order within the types resulting from communications with applicable clinical/project/clerical staff. Collaborates with external service vendors and/or IMITS/Registration Data Quality staff as required.

Participates on internal and external committees, including organizational forms committees as developed. Provides input from a HIM perspective, test scans new forms approved by the committee, including forms with barcodes external to PHC/PHSA/VCH.

Assists HIM Systems staff as required with downtime notifications and provides support for the staff during downtime.

Attends departmental and/or interdepartmental meetings as requested by the HIM Project Coordinator – Document Imaging/Forms Management by methods such as participating in meetings, providing input, and recommendations, and forwarding requests and/or concerns to the Project Coordinator and/or other Leaders as appropriate.

Provides statistical reports as requested by methods such as generating reports/queries
