Summary

Reporting to the Manager, Records Management & Registration, the Clerk IV, Vital Statistics performs a variety of duties related to the processing of documentation related to Birth and Death Notices, answers all queries with regards to the Vital Statistics Registry, assists with the coordination of documentation for the release of bodies to funeral homes/coroner and prepares records for Mortality Reviews. Performs other related clerical duties as required.

Education

Grade 12 and two years’ recent, related experience or an equivalent combination of education, training and experience.

Duties

Reviews for completeness and/or completes documentation related to Birth Notices by performing duties such as verifying paperwork, following up on paperwork deficiencies including missing demographic information, signatures, etc. by checking the applicable charts and/or following up with physician or other clinical personnel to obtain information, entering information on forms, and sending finalized documentation to the Vital Statistics Agency. 

Reviews for completeness and/or completes documentation related to Death Notices and release of bodies by performing duties such as verifying paperwork, following up on paperwork deficiencies including missing demographic information, and signatures, etc. Coordinates the collection of appropriate documentation to Laboratory and/or other designated department required for the release of bodies. 

Organizes and prepares records for the Mortality Reviews/Rounds by conducting activities such as chart deficiency follow-up and ensuring record and documentation completeness. 

Responds to all queries regarding the Vital Statistics Registry in accordance with established procedures. 

Assembles patient records into a standardized order; files delayed (loose) reports. Photocopies prints labels, files materials and performs other related clerical duties as required.

Demonstrates own work procedures to co-workers and students.

Performs other related duties as assigned.
