Patient & Family Centred Care
Providence Health Care is continuing our journey towards patient and family centred care at all of our sites and programs. Patient and family centred care is an approach to the planning, delivery and evaluation of health care that is grounded in mutually beneficial partnerships among health care providers, the people we serve and their families.
We know that to continue providing the best care experience, and to successfully address ever-increasing complexities and future service challenges, we need to work closer than ever with patients, residents and families.
Patient and family centred care is driven by four core concepts:
- Patients and families are treated with respect and dignity
- Patients and families are invited to participate in clinical decision making at the level they choose
- Patients and families are invited to collaborate with us as partners on committees
- Patients and families have access to information so they can be involved in their care
